AFFIDAVIT

I, XXX, son of Z & Y of address ----------------, aged above 21 years, by profession Service, by faith Muslim, by Nationality-Bangladeshi by birth do hereby solemnly affirm and declare as follows:-

01.         That I am a permanent resident and citizen of Bangladesh by birth.

02.         That I was born on 01 Sep 1995.

03.         That my Bangladeshi NID/ Passport No. --------------.

04. That in my said NID/ passport my mother’s name has been erroneously written as “YY”

05. That from now my mother’s correct actual name will be as “Y” (corrected) in my NID/passport. Which I undertake to be used in my all documents and essential papers & new NID/passport.

06. That the affidavit is swearing for correction of my mother’s name before the competent authority.

 

 

That the statement made above are true and correct to the best of my knowledge and belief, in truth whereof, I swear and signed this Affidavit before the 1^st Class Magistrate & Notary public of Dhaka, Bangladesh on-
